Self-Requesting 360 Feedback

Steps for requesting feedback as an employee-level user.

Updated over 2 weeks ago

Employee-level users can request internal feedback for themselves, which means they can only request input from colleagues and team members within the organization.

Self-requested feedback responses are delivered directly to the individual upon submission. Manager users and above can also view any self-requested feedback for their teams.

Steps for Self-Requesting Feedback:

Access the 360 Feedback application by navigating to Apps > 360 Feedback. Click on the green plus button in the upper right corner of the app and select the 'Request Feedback' option.

Once in the feedback request, there are two tabs to be completed. Once each tab is complete, you will click “Request” in the top right corner.

Questions Tab: This section allows you to select from a list of templates provided by your account admin(s) or create a set of questions from scratch.

  • If you select a template, the information will pre-populate and it will not be editable. You will need to select a due date for the responder before moving to the next section.

  • If you decide to create a question set from scratch, you will be prompted to enter the following information:

    • Due Date for responder

    • Optional instructions/guidance for the responder. This information will populate at the top of the 360 form before the questions.

    • You may have the option to ask the evaluator for feedback on the user’s Objectives and Core Values from the current Performance Review (if one is actively assigned). These options may be hidden based on an account setting.

    • If desired, select a 360 project from the dropdown menu.

    • Enter your questions

      • You can choose from three types of questions: Likert, text, or a combination of both.

      • The Likert scale ranges from "Strongly Agree" to "Strongly Disagree."

      • You can add a maximum of 20 questions.

Feedback From Tab: Select who will be providing feedback about you.

  • You can ask anyone in your organization to provide feedback for you. However, the application will filter for your team members by default. If you wish to ask for feedback from someone outside of your team, toggle the "My Team Only" filter off.

Important Note: Manager-level users and above will follow a slightly different process for self-requesting 360 feedback due to their level of access. These users will see an additional tab titled "Feedback For" and must select themselves in this section. Once your feedback is requested, you have the ability to send reminders, delete the feedback requests, and view results. To learn more about accessing and tracking 360 Feedback requests, please view this article.
