360 Degree Feedback templates can be created by Admins or Super Users. In some cases, managers may also have this ability, depending on account settings. While managers might be able to assign templates, creating them depends on whether this setting is enabled. It is recommended for your organization to define and communicate template creation responsibilities to the relevant stakeholders.

To create a new 360-Feedback template, follow these steps:
1. Navigate to Templates > 360 Feedback in your left navigation menu. Click the green plus icon in the upper right corner to add a new template.
2. Give your 360 template a title and complete the optional information at the top of the template creation form:
Position, Department, and Location: These are optional fields that serve as additional labels but do not restrict your ability to assign this template to users outside of the selected criteria.
Project: This field serves as a "tag" for your feedback form. To learn more about 360 projects, visit this article.
Instructions/Guidance/Details: This text area allows you to enter a statement that the responder will see at the top of the 360 Feedback form above the questions that they will be answering.
3. You have the ability to add up to 20 questions to your 360 template. The form will pre-populate 3 questions but you can add more by clicking the teal plus sign.
You will have the option to select a question from your library or create a new custom question. Once you have selected a question, choose the response type you want for this question. You have 3 options:
Text Response: This option gives the responder an open-ended text box to type their response to the question.
Likert: This option presents the responder with a scale that ranges from strongly agree to strongly disagree.
Likert with Text: This option provides the responder with both the Likert scale and an open-ended text box.
4. At the bottom of the template, you may see the option to include performance objectives and core values/workplace behaviors on the 360 form. This will be dependent on your account settings. If checked, this will allow the responder to both rate (on a 5 star scale) and comment on the evaluatee's performance objectives and/or core values associated with this user. This information is pulled from the most recent Performance Review that is associated with the evaluatee in the system.
Note: If your organization does not use the Performance Review application these items cannot be included on the form. Additionally, if the evaluatee has never been assigned a Performance Review, then this information will not be included on the form for this user even if selected.
5. Last, determine if you would like this template to be accessible to employees when self-requesting feedback. Clicking this option will allow employees to select this template from a drop-down menu during the requesting process. Only Admins, Super Users, and Partners have the option to grant employees access to templates.
6. Once all information is entered into the template form, click the Save button in the upper right corner. Templates do not have autosave functionality so the save option must be clicked.

To assign a 360-Feedback template:
Navigate to Templates > 360-Feedback.
In the 360-Feedback Template section, click the three-dot menu next to the template title and select Assign 360-Feedback Template.
For Internal Feedback Templates:
In the first tab, titled Internal 360 Feedback Template:
Select a user from the Feedback For dropdown menu. You can select more than one user. This user is the evaluatee, who the feedback is regarding. Depending on your user role, your options for this tab may be limited.
Next, select a due date for the responder.
In the second tab, titled Feedback From:
Select who will be providing feedback. The people selector for who the feedback is from will include everyone in the company. Click here for questions regarding the People Selector.
After selecting the evaluators, confirm the total number of selected users at the bottom of the screen and click Assign.
For External Feedback Templates:
Select a user from the Feedback For dropdown menu. You can select more than one user. This user is the evaluatee, who the feedback is regarding. Depending on your user role, your options for this tab may be limited.
Select a due date for the responder.
Since this is an external request (someone outside of your organization or WorkDove system), this assignment will send an email to the user who will be providing the feedback. You can send the form to more than one user by clicking the teal plus sign icon to add additional responders.
In the Company Name field, enter the name of the organization or the name of the person responding. This will populate into the greeting of the email (e.g., "Hi {insert name},").
In the Company Email section, enter the email address of the individual at this company from whom you would like feedback.
Once all fields are completed, click Assign.

To edit a Performance Review Template:
Navigate to Templates > 360 Degree Feedback via the left navigation menu.
Click the three-dot menu next to the desired template.
Select Edit Performance 360 Degree Feedback.
Once edits are complete, click Save.
Important Note: Edits to a 360-degree feedback template do not automatically apply to any pending or previously submitted 360 requests. Changes will only take effect for future requests created using the updated template.
